Registration Instructions-
- Download & complete application.
- Bring the following things with you at the next sign ups.
- Copy of Birth Certificate
- Copy of Insurance Card (front & back)
- Copy of Court Order (If applicable, i.e. custody, guardian)
- Copy of physical (must be within last year)
- Check for $75.
